- Original High School Card (for HS graduates)
- Transfer Credentials (for college-level or college graduates)
- Original Good Moral Certificate
- Original PSA Birth Certificate
- Medical Certificate
- Two (2) pieces 1×1 ID picture
- Three (3) passport-size ID photos (white background, collar shirt with name tag)
